 1. Project Background and Rationale
The Project Punaraya aims to cover the whole spectrum of service delivery components i.e., infrastructure, equipment, motivation, skills, monitoring, and increasing demand from the community side thereon access to quality health services having the focus on the target group of maternal & child health and adolescent health. The project would have equal weightage to all components and cross-cutting factors that are directly connected to overall quality service delivery.
At the beginning of the project, it is planned to conduct a comprehensive health profile assessment to understand the health challenges and service gaps in the selected geography. This assessment will identify prevalent health issues, including communicable and non-communicable diseases, maternal and child health concerns, and nutritional deficiencies. It will also evaluate the availability, quality, and accessibility of primary healthcare services, including essential medicines, diagnostics, and referral systems. The assessment will further explore risky health behaviours such as tobacco use, alcohol consumption, and delays in seeking care, alongside barriers to accessing formal healthcare services. Based on these findings, actionable recommendations will be developed to address gaps, improve health behaviours, and strengthen healthcare infrastructure.

- Conduct a baseline health facility assessment to map existing services, gaps, and resource requirements.
- Assess community health concerns related to anemia, maternal, child and adolescent health, and non-communicable diseases (NCDs).
- Come up with evidence-based recommendations to increase footfall and utilization of health services.

- Geography of the assessment:
The geography of the assessment would be under the of district of Kamrup Rural in Assam as follows:
- Palasbari SD (State Dispensary) and
- Bazrapara AAM (Ayushman Arogya Mandir)-under Kamrup (Rural)-Rampur Block
- Barkuchi AAM (Ayushman Arogya Mandir)-under Kamrup (Rural)- Chayani Borduar Block
- Key Tasks
B1. Facility-Level Assessment
- Review of service availability (maternal & child health, adolescent health, NCD screening, anemia management, etc.).
- Gap analysis of infrastructure, human resources, essential medicines, and equipment.
- Evaluation of record-keeping, available digital systems considering Ayushman Bharat Digital Mission (ABDM)
B2. Community Health Assessment
- Household surveys on health-seeking behavior and practices.
- Focus group discussions with women, adolescents, and with male and female of NCD age groups.
- Identification of barriers to accessing public health services (distance, awareness, cultural factors).
- Mapping perceptions around anemia, NCDs, maternal health, and adolescent health.
B3. Strategic Recommendations
- Proposals for infrastructure and cosmetic improvements under phase 2.
- Recommendations for essential equipment and service additions.
- nterventions to increase patient footfall -community outreach, IEC/BCC campaigns etc.
