India grapples with a significant burden of non-communicable diseases (NCDs), prompting government initiatives like the National Program for Prevention and Control of Non-Communicable Disease (NPNCD) since 2010. Despite efforts, challenges persist in fully utilizing primary health centers (PHCs) and sub- centers (SCs) due to infrastructural limitations and workforce shortages.

1. Target Beneficiaries: Community members aged 30 years and above, including frontline healthcare workers at health centers.
2. Target Audience: All citizens aged 30 years and above, as well as health teams and frontline healthcare workers.
3. Project Areas: PHC Maan in Pune and UPHC Mullahera in Gurugram.
4. Donor: Boston Scientific.

On World Hypertension Day 2023, the Government of India unveiled revised guidelines for the NPNCD, targeting 75 million individuals with hypertension, diabetes, or both by 2025. This ambitious endeavor aims to provide standard care at primary healthcare levels, marking a significant stride in NCD management globally.

Amidst these challenges, there’s a pressing opportunity to bolster infrastructure, supplies, and human resources at health centers and drive community engagement for NCD services. The People to People Health Foundation (PPHF) has stepped in to bridge these gaps, enhancing NCD service delivery efficiency and community mobilization efforts.

Program Objectives

1. Goal: Address primary healthcare needs for NCDs in PHC and UPHC areas for underprivileged
populations.
2. Increase public awareness through community health education about NCDs.
3. Strengthen implementation of NPNCD population-based screening guidelines at PHCs and UPHCs.
4. Enhance the capacity of PHC and UPHC health teams to provide essential NCD services effectively.
