India shoulders a significant burden of non-communicable diseases (NCDs), prompting the Government to initiate the National Program for Prevention and Control of Non-Communicable Diseases (NPNCD) in 2010, supplemented by guidelines for population-based screening in 2016. However, primary health centers (PHCs) and sub-centers face challenges in providing optimal care due to infrastructure deficiencies, limited resources, and geographical constraints.

On World Hypertension Day in 2023, the Government revised NPNCD guidelines, targeting 75 million individuals with hypertension and diabetes by 2025. People Health Foundation (PPHF) seized this opportunity to bridge gaps in NCD service delivery, exemplified by the SCREEN project. In collaboration
with Wipro GE Healthcare and five state governments, SCREEN 1.0 capacitated over 4,700 healthcare providers, screening 45,200 individuals across 797 camps. Building on this success, SCREEN 2.0 extended efforts in Assam, training 571 healthcare workers and screening 22,000 individuals in four PHCs. These initiatives reflect a concerted effort to combat NCDs and enhance community health infrastructure.

Program Objectives

Goal: Meet primary healthcare needs for NCDs in PHC areas.
Objectives:
1. Raise public awareness of NCDs through community health education.
2. Improve implementation of population-based screening guidelines at PHCs.
3. Enhance the capacity of PHC health teams to provide essential NCD services.
