Subject: Proposals invited for Comprehensive Infrastructure Assessment of 16 health facilities (PHC/HSC) in Kamrup (Rural) District of Assam

You are invited to submit your Detailed proposal including Approach, Methodology, Deliverables, Timelines and Financials component.

Objective: To conduct a Comprehensive Infrastructure Assessment of 16 health facilities (PHC/HSC) in Kamrup (Rural) District of Assam

Scope of work

1. Detail Assessment of PHC/ HSC in terms of Infrastructure, Service, Equipment as per IPHS guideline.

2. Assess the engagement of Rogi Kalyan Samity for strengthening non clinical services and processes including waste management, sanitation & hygiene, infection control fund creation and others. The scope of work includes engaging with the Rogi Kalyan Samiti to understand how their role can be factored in for institutionalizing the integration of the expected healthcare service delivery through their involvement to make sure the program is sustained with community involvement.

3. Preparation of Action Plan / Report facility wise with Budget to make a model PHC/HSC in the district.

Timeline: 45 days including assessment and submission of the final report
