Introduction

PEOPLE TO PEOPLE HEALTH FOUNDATION (PPHF) is a non-government organization committed to transforming lives for improved health and wellbeing through locally driven solutions. PPHF have been working with Ministry of Health and private sectors in India to address the health program and policy implementation gaps. PPHF has very intensive experience in building capacity of health system and mobilize community for uptake of better health services. PPHF provides technical support on quality public health delivery, capacity strengthening and health technology. Besides that, PPHF generate new knowledge through implementation and demonstration for better programming and scaling up and has expressed its willingness to act as implementation partner in the execution of the Project.

Instructions to the Agencies

1. Re-submission of Proposal

a. The agency can only submit one proposal at a time.

2. Completeness of Response:
a. The agencies are requested to study all instructions, forms, terms, requirements and other information in the RFQ document carefully (including BOQ Format). The proposal should be strictly based on the attached BOQ format. Submission of the proposal shall be deemed to have been done after careful study and examination of the RFQ document with full understanding of its implications.
b. The response to this RFQ should be complete in all respect. Failure to furnish all information required by the RFQ documents or submission of a proposal not substantially responsive to the RFQ documents in every respect will be at the agency’s risk and may result in the rejection of its proposal.

3. Agency Inquiries/Clarifications:
a. The agency shall send their queries to connect@pphfglobal.org with the subject PPHF PHC Medical Equipment – RFQ, before the due date for submission of queries/clarifications as mentioned in the key events and dates.
b. Queries on the telephone, social media, or any other medium will NOT be entertained.
c. PPHF will facilitate, in case the agency wishes to visit the site for assessment.

4. PPHF’s right to terminate the process:
a. PPHF may terminate the RFQ process at any time and without assigning any reason thereof.
b. PPHF reserves the right to amend/edit/add/delete any clause. However, this will be informed to all and will become part of the RFQ.
c. During the RFQ process, if any information is found false/ fraudulent/ mala fide, then PPHF shall reject the proposal and if necessary, initiate appropriate action.

5. Language of Proposal:
a. Proposals should be submitted in English language only.

6. Documents (Scan) to be submitted and Pre-qualification criteria:
a. The Agency should be an agency registered in India.

Documents to be submitted: Certificate of Incorporation/Registration, PAN Card, GST Certificate, Previous Work Experience Certificate & Related Reports.

b. The Agency will be expected to have a minimum experience of 5 years in a similar area.

c. The Agency should not have been blacklisted by any Government Agency/PSU/Central/State agency or under a declaration of ineligibility for fraudulent or corrupt practices.

Documents to be submitted: Declaration from the currently authorized signatory of the company.

7. Evaluation Process:
a. PPHF will shortlist the proposals based on the Shortlisting criteria.
b. Internal Committee of PPHF shall evaluate the shortlisted proposals based on procurement policy.

8. Penalty
a. PPHF shall be at liberty to impose penalties, including cancellation of the contract if the agency indulges in fraudulent activities, malpractices and male fides.

9. Non-Disclosure Agreement (NDA)
a. Selected agencies will have to sign the Non-Disclosure Agreement (Annexure) with PPHF.

10. Sub-Contracting of Agreement or Purchase Order
a. The requested assignment can’t be assigned or subcontracted to another agency.

11. Proposal Preparation Costs
a. The agency shall submit the proposal at its own cost and PPHF shall not be held responsible for any cost incurred by the agency. Submission of a proposal does not entitle the agency to claim any cost and rights over PPHF and PPHF shall be at liberty to cancel any or all proposals without giving any notice. All materials submitted by the agencies shall be the absolute property of PPHF and no copyright/patent, etc. shall be entertained by PPHF.

12. PPHF’s right to terminate the process
a. PPHF may terminate the RFQ process at any time and without assigning any reason thereof. PPHF reserves the right to amend/edit/add/delete any clause of this Proposal Document. However, this will be informed to all and will become part of the RFQ.

13. Submission of Proposals
The proposal submission shall comprise of the duly filled as per BOQ format attached with RFQ.

14. Late Proposals
Any proposals received by PPHF after the deadline for submission of proposal shall be declared late and will be rejected and returned unopened to the agency at the discretion of PPHF.

TERMS OF REFERENCE

Purpose: Purchase of Medical Equipment for Urban Maternity Centre in Doddatumkur PHC, Bangalore Rural, Karnataka

Period of Performance: 30 Days from the signing of the agreement or Purchase Order

1. EXPECTATIONS FROM THE BIDDERS
• The Agency will report to the Activity Manager and will work directly with the PPHF team. The agency is required to depute a resource person for day-to-day work/updates and revisions.
• The agency will help the team by working at shorter deadlines. A prior experience of working with Government institutions is preferred. The agency is expected to have flexibility in terms of turn-around times.

2. RESPONSIBILITIES/SCOPE OF WORK

For this, under the direct supervision of the Activity Manager or the PPHF representative(s), the agency will be required to do the following:
• Take necessary briefs from the project team at PPHF

3. QUALIFICATION/CRITERIA FOR SELECTION:
The agency applying for this must have the following:
• Located/ work experience in Karnataka.
• Minimum 5 years experience working in supplying medical equipment to Govt. facilities like hospitals, PHC’s etc.
• Extensive experience in the maintenance of medical equipment.
• Local knowledge of culture and languages in Karnataka.
• Experience with similar assignments in Karnataka.

Note:
1. The cost should be actual & should be quantified against each head.
2. The quotation should be on the company’s letterhead with a sign and company’s stamp and in PDF form.
3. Cost should be based on the specification mentioned in the BOQ.
